Getting an Italian Driver's License as an American Resident (2026)

By Steven DavisFacts checked October 24, 2026

The short answer

A US license works in Italy only with an International Driving Permit or official translation, and only for your first year of residence. Italy converts US licenses only for diplomatic staff and their families, so other residents must earn an Italian patente (driver's license) by exam. That means a medical visit, a theory test in Italian, French or German, at least 6 hours of driving-school lessons and a road test. Official fees total €74.40, plus the doctor's fee and any driving school.

Years of driving in the US don't shorten the process: a US license holder takes the same theory exam and road test as a first-time driver.

How long your US license works in Italy

Article 135 of the Codice della Strada (Highway Code) sets two conditions for driving on a non-EU license. You carry an International Driving Permit (IDP) or an official Italian translation with it. And you have not been resident in Italy for more than 1 year. Both documents must be valid.

Driving without the IDP or translation costs €408 to €1,634. The rule also covers visitors: see the International Driving Permit guide.

The permit must come from the country that issued your license. In the US, only AAA and the American Automobile Touring Alliance (AATA) are authorized by the State Department to issue it. An IDP is valid for 1 year and cannot be renewed, so get it before you leave.

When the year starts. The law counts from the day you acquire residenza anagrafica, your registration at the comune's anagrafe (registry office). Your arrival date and your visa date do not start it. See registering residency.

What happens after the year. Driving on your US license then brings a fine of €158 to €638. The officer withdraws the license and sends it to the prefect, who returns a non-convertible license to the authority that issued it. The same fine applies if your US license expires during your first year, so renew it before you move.

Why you can't convert a US license

A foreign license can be swapped for an Italian one only if its country has a reciprocity agreement with Italy. The Ministry of Infrastructure and Transport (MIT) list, updated October 9, 2026, includes Japan, South Korea, Brazil, Switzerland and the UK. Albania and Uruguay join for applications from November 2, 2026.

The United States appears only in a separate category: licenses held by diplomatic staff and their families. No individual US state is listed. (An older list, posted on an Interior Ministry prefecture site in January 2024, also names consular staff.) For everyone else, the MIT page is plain: you get an Italian license by exam.

The steps to an Italian patente

From medical visit to license: 5 steps, each with a legal deadline or minimum wait
  1. 1Medical visitCertificate valid 3 monthsAn authorized doctor checks that you are fit to drive and sends the certificate electronically.
  2. 2ApplicationOpens a 6-month windowForm TT2112 at the Motorizzazione Civile, filed by you or by a driving school.
  3. 3Theory examWithin 6 months of applying30 true-or-false questions in 20 minutes, up to 3 errors. 2 attempts, at least 1 month apart.
  4. 4Foglio rosa and lessonsPermit valid 12 monthsLearner's permit, plus at least 6 hours at a driving school: motorway, extra-urban roads and night driving.
  5. 5Road testFrom 1 month after the foglio rosaA vehicle check, maneuvers, then driving on the road. Up to 3 attempts, at least 1 month apart.

The law sets deadlines and minimum waits. Exam dates depend on your office's booking calendar.

  1. Medical certificate. Book a visit with an authorized doctor, such as your ASL's (local health authority's) medical-legal office. Bring a photo, a €16 stamp-duty receipt paid through PagoPA, the doctor's fee receipt and your own doctor's certificate of past illnesses. The doctor sends the certificate electronically; it is valid for 3 months.
  2. Application. File form TT2112 at the Motorizzazione Civile (provincial motor vehicle office). Attach a photo, copies of your ID, codice fiscale and permesso di soggiorno (residence permit), the medical receipt and the fee receipts. If your permit has expired, add the renewal receipt.
  3. Theory exam. You have 6 months from the application to pass it, with 2 attempts. Per the Friuli Venezia Giulia Motorizzazione (August 2026), it has 30 true-or-false questions in 20 minutes, with up to 3 errors allowed.
  4. Foglio rosa and lessons. Passing the theory exam gets you the foglio rosa (learner's permit), valid for 12 months. You then need at least 6 hours at an autoscuola (driving school): motorway or main extra-urban roads, other extra-urban roads, and night driving. Lessons are one-to-one, at most 2 hours a day.
  5. Road test. It comes at least 1 month after the foglio rosa, in a dual-control car. An examiner has you check a vehicle part chosen at random, do maneuvers, then drive on the road. You get 3 attempts, at least 1 month apart. Pass, and the office issues your license.

If you fail all 3 road tests, you can request a second foglio rosa once, keeping your theory pass. Trento's office requires the request within 2 months after the first permit expires.

Which language the theory exam is in

The exam is in Italian. Since January 17, 2011, you can ask for French or German anywhere in Italy, under an MIT circular. No other language is allowed, even for a fee, as a Senate bill (no. 1543) summarized in June 2025.

That bill proposes an English option; as of October 24, 2026, the Highway Code has none. Audio support and extra time exist only for candidates who qualify, such as people with a learning disorder. See learning Italian.

Driving school or privatista

With an autoscuola, the school files your application and books both exams. The official tariffs don't cover school fees, so ask for a written quote that lists lessons, exam fees and paperwork.

As a privatista (independent candidate), you apply at the Motorizzazione yourself and book the theory exam on your own. Ask your office how it takes bookings: Trento's, for example, works online with SPID or the CIE (electronic ID card). You still need the 6 hours at a school, and Trento says privatisti must enroll there for the road test too.

Outside lessons, you can practice beside a companion aged 65 or under, licensed for the category for at least 10 years. That is allowed only after the motorway, extra-urban and night lessons, and you must carry the school's certificate. The car needs a "P" sign. Practicing without a qualifying companion costs €430 to €1,731 and a 3-month impoundment of the car.

Driving schoolNot in the official tariffsAt least 6 hours of lessons required

These are the national tariffs on the MIT's patente B page (updated November 13, 2025). Sicily, Valle d'Aosta, Trentino-Alto Adige and Friuli Venezia Giulia have their own tariff tables. The Trento and Friuli Venezia Giulia pages show the same amounts.

After you pass

For your first 3 years, the new-driver limits apply regardless of experience abroad. You may not exceed 100 km/h on motorways or 90 km/h on main extra-urban roads. You also cannot drive cars over 75 kW per tonne, or over 105 kW for passenger cars.

A category B license is valid for 10 years. If you are over 50 when it is issued, it lasts 5 years; over 70, 3 years.

Common problems and fixes

Losing track of the 1-year deadline. The law counts the year from your anagrafe registration, not from your arrival, visa or permit. Note that date and apply soon after it: the theory window is 6 months, and exam dates depend on your office's calendar.

The technical Italian of the theory exam. Newcomers with conversational Italian report that the exam's wording is harder than expected. Study with Italian-language practice quizzes from the start, well before your first-year deadline.

Not knowing how to book as a privatista. Independent candidates report confusion over how their office schedules the theory exam. Ask when you file the application, and check your office's page. If the process stalls, a driving school can take over the booking.

Do you need help with this?

Most residents who read some Italian can handle the paperwork alone, especially through a driving school. It gets harder when your first-year deadline is close or your Italian is still basic.

Frequently asked questions

Can I convert my US driver's license to an Italian one?

No, unless you are a US diplomat or a family member of one. Italy converts foreign licenses only from countries with a reciprocity agreement, and the Ministry of Infrastructure and Transport list (October 2026) includes the United States only for diplomatic staff and their families. No individual US state appears on the list. Other residents must take the Italian exams.

How long can I drive in Italy with a US license?

Until you have been a registered resident for 1 year, counted from your registration at the comune's anagrafe. You must carry an International Driving Permit or an official Italian translation with your license. After the year, driving on it brings a fine of €158 to €638 and the license is withdrawn.

Can I take the Italian driving test in English?

No. The exam is in Italian, or in French or German on request. A Senate bill filed in June 2025 proposes an English option, but as of October 24, 2026 the Highway Code does not include it.

How much does an Italian driver's license cost?

Official fees total €74.40 in October 2026: €16 stamp duty for the medical visit, €26.40 plus €16 for the application and theory exam, and €16 for the road test. The doctor's fee and driving-school lessons are extra and not covered by official tariffs.

Do I have to go to a driving school?

Partly. Everyone needs at least 6 hours of lessons at a driving school, covering motorway, extra-urban and night driving. You can file the application and take the theory exam as an independent candidate, but Trento's office says independent candidates must enroll at a school for the lessons and the road test.

Do new-driver limits apply if I have driven for years in the US?

Yes. The limits apply for 3 years from when you get the Italian license: 100 km/h on motorways, 90 km/h on main extra-urban roads, and no cars over 75 kW per tonne or 105 kW for passenger cars.
